All Returned Materials Authorization requests must follow the outlined process & contact procedure
A) Returned Materials Authorization numbers can be requested in a variety of ways. Please select from one of the below contact methods:
1) Online Requests can be submitted via the RMA Online Submission Form.
2) Please fax the Returned Materials Authorization form to (419) 869-1926
3) Contact the RMA Administrator Pat Dull: (419) 869-1826
After request submission the RMA Administrator will contact you and issue the authorization number
Please note: No Return Goods Will Be Accepted Without a Returned Materials Authorization Number or Returned Materials Authorization Form
B) After the authorization number is assigned, please ship the goods to:
